| PREPARATION OF YOUR HOME |

So our Cleaning Specialists can focus their energies on cleaning, which will maximize the cleanliness of your home or office we ask that you take a few minutes the night before your scheduled cleaning to tidy things up. We appreciate your cooperation.
LOCK-OUTS: Many of customers give us the key to their home or office. If for some reason you elect not to give us a key and we do not have access to your home or office when we arrive a lock-out fee will be charged. Also, forgetting to call our office to reschedule your service or not leaving your key in the designated hiding place, a lock-out fee will be charged.
First Time and One Time Cleanings: We will charge a fee of $50. For any lock-outs or turn a ways on first-time or one-time cleanings.
Repeat Cleanings: We will charge a fee of $40 for any lock-out or turn-away on regular scheduled cleanings. |

| NON-EMERGENCY CANCELLATIONS |
|
| Your Cleaning Specialists are only paid for the work they perform. Please notify the office 48 hours in advance to reschedule or cancel your appointment. Advanced notice enables us to schedule occasional customers into your reserved time. Cancellations made with less than two working days’ notice are subject to a $40 cancellation fee. |

| ADDITIONAL CLEANING SERVICES |
|
| Additional cleaning services (i.e. refrigerator, oven, cupboards, etc.) are available upon request. Please schedule your additional service in advance. Charges for these service are a set amount. |

| PAYMENT POLICY |
|
Payment is due at time of service in the form of cash, check, or major credit card. All First-Time or One-Time Only service are on a cash-in advance or major credit card basis only.
There is a $30 fee for any check returned by your bank. If your bank is in error, they should reimburse you for this expense.
There is a $80 fee, plus any attorney’s fees, in addition to the balance due, for any account we must refer for collection. You further agree to reimburse us for any and all collection or legal fees we incur in collecting this fee.
State tax rate: 7% |

| Platinum Pro-Clean, LLC is licensed, insured and bonded. We will furnish proof upon customers’ request. |

| ACCIDENTS |
|
| We treat your home or office with exceptional care. If an accident or damage occurs we will contact you immediately and rectify the problem. However, we are not responsible for damage resulting from pictures or shelves not proper mounted or loose table and chair legs. |
